Sales invoice not saved - also do not pull the contact and address in the sales invoice

hi

it is not letting us save the sales invoice saying did not save. also it does not pull the customer contact or address into 

also the error of showing the same exchange rate and says the exchange entry for same currency cannot be done:

our version is 3.9.2

Do you have any custom script for Sales invoice or Customer? Looking like it is something related to custom script. Please share the custom script  using github gist, if you have any.

Also please check browser's error console, there should be some error message and share the error message.
