This is done through 'Google Forms' sir. If you are using laptop or desktop, go to your google drive. There is a + icon. click on it. Go to google forms and check all options. If you are trying through android mobile, then you can do it in 

docs.google.com/forms 

and the file will be saved in your google drive.
