We have reviewed the logs on the SMTP server and do not see any login attempts associated with the specified account during the "SMTP Test 8, 14, 18 (Send)." Additionally, there is no record of the email in the Sent folder.
However, we were able to find logs for the "SMTP Test 9, 16, 20 (Receive)" and confirmed the email is present in both the Sent and Inbox folders.

The test is now passing!
We were not actually sending the email. I had misunderstood how the test works and expected the test tool to send the email through our SMTP server and verify that it was received by wellf...@ett.healthit.gov. Thank you for clarifying this.
