Submissions not saving

Hello,

One of our users is having a bizarre issue where when he's working on a submission, this happens:

We can't figure out why it's happening.  It never occurs when I try to submit materials while impersonating him, and it occurs maybe half the time our ScholComm Librarian tries to submit materials while impersonating him.  Any idea what could cause this behaviour?  High server load?  Bad internet connection?  Software glitch?  The poor user is understandably very frustrated.

well…
I don't know anything on your network over there.
University network ? VPN ? External user ?

You should have a look on the server load anyway…AI-scrapers are a
plague today.
Had a look on the log in parallel (web server, pm2 and DSpace) when user
tries to upload (or librarian) ?
Tried different browsers ?
Tried different devices ?
Are they able to open the dev tools in the browser an a have a look on
network and console output ?
Does the librarian have problems on its own account when uploading
something ?
Does it happen on a specific time ?
Can you give the user a second account and look what happens ?

Snake-oil section:
do both use WLAN ? Can you convince them to use cable, just to clarify
that ?
