And I’m not being facetious with the answer below. I was speaking from bitter experience. I recently had an item in DSpace that was acting VERY strangely, to the extent that I wondered if it was being blocked from display by a third party. (!)
After combing through the item authorizations for a second time, I realized that one of the policies was missing. I recreated the policy and the problem was resolved.
