can anyone please help to extract data from the above dashboard?

Looks like the source is directly linked to Ambdavad Municipal Corporation (AMC). They release patient names and address on the website which is coded into google maps.

The maps can be downloaded as kml. Attached is the kml export of the data (updated 27 April) to excel.
