error while filing MGT14 for change of name of company

Dear Members,

While filing form MGT14 for change of name of company it just shows the error message as the screen shot given. After pressing ok, it gives messege as ok, it is not allowing to proceed further for MOA /AOA.

Kindly guide how to proceed further.

Cancel the form and again fill up the form. 
Don't opt for another option except change of name.

MGT-14 can be filed for single purpose only.
