If he is a director of one company, contact ROC, they will give u an affidavit format

Which ROC are you referring to?. 

Also even if the person is not a director. but his disqualification tenure of 5 years is over.

Is there any procedure wherein by submission of an affidavit we can re-activate the DIN.

Am talking about chennai ROC, today only I get to know this from Chennai ROC.

If he is appointed as a director in only one company then affidavit is enough to revive his DIN

Alternatively you can file a writ petition seeking relief and submit the petition copies, along with order copy and affidavit to respective ROC, who disqualified the director by issuing notice of strike off and issued disqualification notice.

Then attach DIN status and submit the same with ROC, they will forward to e-governance, DIN Cell.

Once this is received by them, within 10-15 days TAT, DIN can be reactivated.

This is my experience with ROC-Chennai.
