provide me the format-nidhi limited

Dear All, 

Please provide me the format, 

Declaration from all the subscribers should be attached stating that rule 4, 5, 6 and 10 of Nidhi
Rules, 2014 shall be duly complied with.
As per proviso to Rule 12 of Companies (Incorporation) Rules, 2014, in case
pursuing of any of the objects of a company requires registration or approval from sectoral regulators such as RBI,
SEBI, IRDAI etc., registration or approval, as the case may be, from such regulator shall be obtained by the
proposed company before pursuing such objects. A declaration in this behalf is required to be submitted.

Brijesh Sharma ji, what is the status of your Nidhi registration ? Any one recently did Nidhi registration because MCA is not even considering the response and asking for resubmission again.
