As per canara Bank HO memo 3/2021 , you have to submit the medical expenses application to the concerned CO where the pensioner resides. But you can submit the scanned copy of the application ( duly filled and signed) to the concerned CO by e mail to HRM section.,though it is not mentioned in the memo

I have submitted today the annexure-5 duly filled up and signed to HRM section,Hyderabad circle,
in my email to hrmc...@canarabank.com.
Branches were reluctant to give acknowledgement.
Earlier sometimes, the form submitted at branch level used to miss and we have to approach again
the branch and we have to request about tracing the status.
Now one can submit the scanned copy of annexure-5.
 in email id, to  one's respective circle office though it is not mentioned clearly in 
H.O.Memo 03/2021

Instead of wasting manpower,manhours, stationery and what not, why don't they credit the staff welfare expenses as soon as they submit the life certificate or any specific date convenient to them ?

Yes amount eligible can be credited on a certain day say pongal . Thus unnecessary work can be avoided to staff as well as the exbankmen. I suggested to our Bank indian bank management . But it is invein
