Also, what church is the mother from? Nossa Senhora da Assunção?

There is archaic spelling throughout these entries and some of the children throughout the pages have first and middle names given. I would assume (using modern spelling) that his name is André José. Dad is native of São Pedro and mom is from Matriz Nossa Senhora de Assunção both on the island of Santa Maria.

Here is my read of the record as written…

“Ander Joze filho de Antonio Cabral natural desta Parochial do Apostolo Sam Pedro desta Ilha de Santa Maria, e de Maria de Jesus natural da Matrix de Nosa Senhora da sumpsam da mesma Ilha, nasceo em os vinte sinco dias do mes de Novembro do anno de mil e sete centos, e sincoenta e outo annos; e foi baptizado nesta dita igreja do Apostolo Sam Pedro Parochial de seos pais pello Reverendo vigario della o Lecensiado Lourenco de Frias Coutinho em os tres dias do mes de Dezembro do ditto anno e foi Padrinho o Alferes Manoel de Moura Souza homen cazado morador nesta Parochial, e testemunhas que comigo asignaram com o Reverendo vigario os padres Manoel Frenandes de Mello e Joze Ignacio da mesma Freguezia. Era Supra”

Here are the key elements of the record...

André José (modern form), born in São Pedro, Santa Maria The child is the son of António Cabral (São  Pedro, Santa Maria) and Maria de Jesus (Matriz - Vila do Porto, Santa Maria). The child was born on November 25^th 1758 and baptized on December 3^rd 1758 in Igreja Paroquial do Apostolo São Pedro, Santa Maria.

The parents of the child had a number of other children (possibly 10 or more in total). In the baptismal records of the other children, the recording of the name of the father varies and includes Antonio Cabral, Antonio de Melo and possibly Antonio Maria de Melo. A bit more work is needed to confirm that these are indeed the same individual.
