您好，不好意思，我已經於2021/5/12在表單中進行回報，然而至今尚未收到確認信，因此想問一下，是有資格不符還是上傳錯誤等其他問題，謝謝。

您可以確認信件是否在不同的分類夾或是垃圾信中，

若您仍然沒有收到，可能是您並未報名活動，系統無法發出信件聯繫您，謝謝
