URGENT: Submitting a form for developer token gets error 500 internal server every time I click on the submit button

Anyone has the same problem with me? or Can someone HELP me to fix this error?

I have already submitted a form to get the developer token, but every time I click on the submit button it shows up the error message "500 Internal Server Error". I have checked the file size, but it is still pretty small, which is only around 2MB and I have already changed the permission of the file to 775. I read online that someone can fix it by clear the cookies and cache from the browser, but still did not work for me.
