Conversion tracking via API

Hello,

Recently I started using AdWords API, and I have two issues related to recording conversions:

1. When I send API request to update conversion I received back response with status "success".
Unfortunately I can't see most of the conversions in the UI although the "success" was reported.
Do you have any idea why this happend too often?

2. from time to time I received error "TOO RECENT CLICK" although that I'm waiting more then 6 Hours between the click and conversions.
I'll like to know what is the rule for the updating conversions via API? and what is actually the event when you start to count the 6 hours?
