Google Ad is not performing after 2 weeks

Team,

We are running google campaign for last 6 months to generate leads. Whenever running a new campaign, it is giving leads for 2 weeks. After two weeks, it is not generating single lead.

Campaign Type: Search

Ad Type: Lead Ad

Strategy: Initially Maximum Clicks then we will change maximum conversions. 

If I modify the campaign again, it will generate leads. Is there any mistakes we are doing. Can anyone suggest us?
