API Annual Survey Question Summary

Hi! My team and I would like to fill out the annual API survey, but want to gather a list of questions in advance so we can best organize our answers. Can you please share a document that summarizes the survey questions?

For context: The survey is not delivered via a Google Form, so it's not possible to easily gather a list of questions and return to the start of the survey.
