Can experts please give me a sample form to successfully register for the basic API package? I registered according to the schedule but was not successful in any of them.

You may fill out the basic access application form to obtain basic access for your manager account. If you're still unable to submit the form, I would recommend you reach out to the Google Ads API Compliance team as they are better equipped to address your concerns.
