Many comments on this crash blame the Dutch pilot but something called the heterodyne effect made him think he had been given the OK to take off.

Concentrating on his take off in thick fog and under pressure to meet a deadline, he did not give full attention to a warning from the junior crewman at the very last moment. He did attempt a memorable evasion that, because of the extra fuel he had on board, failed.
