शिवं धर्मार्थमोक्षाय चतुर्वर्गाय चण्डिकाम् ||

This verse is found in Padmapurana 1.49.65 (srishti Khanda, chapter named Sadachara Varnana)

आरोग्यार्थंचसूर्यंचधर्ममोक्षायमाधवम्

शिवंचकृत्यकामार्थंसर्वकामायचंडिकाम् ६५

Bhaskararaya has quoted this verse in his commentary on Lalita Sahasranama Phalasruti.
