KDE menu bugs

I've encountered a couple KDE menu bugs:

- Creating an AppVM with the same name as a previously-deleted AppVM
results in no KDE menu item being available for the newly-created AppVM.

- Sometimes, AppVM menu items fail to get updated icons (e.g.,
old-style lock icon rather than flat new lock icon; old Qubes logo
rather than new Qubes logo) and become stuck with the old icons or no
icon at all. (In the case of the new lock icons, there seems to be no
way to manually specify the new lock icon within the KDE menu settings.
Only the old-style lock icons are available.)

Which is KDE problem (no way to invalidate icon cache).
