TOC not generating

Forgive me if I already sent this message but I do not see it listed in the topic list.
I am using MW 1.18.+ on a windows box. (not that it matters)
I have mwlib 0.12.10 on a redhat E5
I have great PDF's being generated with multiple articles with our logo on the cover. Why would the TOC not be generated?
