What do you use to generate graph images in docs?

I would like to generate similar graph images, what tool do you use to create your graph images in documentation.

Like the following:

That specific image was done with omnigraffle. It's not "generated" - it's a hand done layout and not backed by data. You'd need a different tool for dynamic graph visualization like Gephi or Cytoscape if you wanted something auto-generated
