I couldn't find any info on this.

I created an icon and Gm downloaded it.
But then I changed the icon and Gm didn't notice.
I deleted the image Gm downloaded and Gm just showed nothing.

Does Gm check the downloaded icon against the original and re-download it?

I'm guessing it doesn't due to speed and processing concerns.

I didn't see any way to refresh the icon.
You would think Gm would at least notice that the file is gone and re-download it.

But having to go in and delete the image is a bit much usability-wise.

You need a context menu right-click on the icon Refresh, or something.

Unless I'm missing something.

(the work-around was for me to manually save the new icon in the directory,
overwriting the file that was there -- a pain for non-Power Users)

Not unless the script changes.  If you change anything about your script, including the @icon, @requires, @resources, etc: increment the @version, and the auto update will figure it out.
