Issues creating an application

I have recently tried to create the first application on a new account
and run into issues. I went through the standard registration process
for the application and entered the SMS'd PIN to finalize the creation
of the application. However, the application never showed up in my
list of managed applications. Since this happened, every attempt to
create another application allows me to enter the new application name
into the first request window, but then redirects me back to the main
admin page with no applications listed. If anyone has run into this
issue or knows what I should do to resolve it, I would greatly
appreciate your assistance.
