Missed Midterm IP Address Form Deadline

Question:

Unfortunately I missed the deadline to fill out the IP address form as I didn't notice the posting in the google groups. Is it still possible for me, and others in my position, to take the midterm? If not could I send you my IP address through some means other than the form?
