This is a critical reminder that our End-of-Year (EOY) Package is due next Monday, June 15, 2026. To ensure the timely release of our Grant Award Notifications (GANs) for the next fiscal year, it is imperative that all Region 5 projects submit their reports electronically by this date.
⚠️ Important: Do not wait until the last minute! The database runs very slowly when everyone tries to input data during the final week. Reports must be fully submitted electronically to the CDE by June 15th, not just in progress.
Please log into the CDE Reporting page and verify that the following 5 components are complete:
Annual Program Report: Verifies contact info (Page 1) and actual numbers served/placed versus funded targets (Page 2).
2026-27 Initial Grant Budget: Your best projection for next year's spending. Make sure line-item totals on Page 3 match Page 2 exactly (if numbers are red, they do not balance). Don't forget to include student wages for work experience.
Annual Program Requirements Report: Formerly the Ed Code Requirement Report. Use MS Word or Google Docs to type and spell-check your narratives (350-word max per box) before pasting them into the database to prevent losing work from system time-outs.
Student Baseline and Follow Along Data: Ensure all baseline records are complete (no blank fields, school of attendance entered, and leavers exited).
Submit Follow-Up Data: Confirm all follow-up questions have been answered so that no student names remain on the left-hand side.
Remember, nothing is officially sent to the CDE just by marking items "Ready for Submit."
The Project Contact must click the "Send to CDE" button. If you don't click this button, your package will be late!
I will personally be out of town starting today, June 8th. If you experience issues or have questions, please follow the protocol below.
