The form has issues - some of the fields which ought not be required fields are required fields. I've mailed GSCASH. I hope they'll fix it, and I advice everyone to stand by until the corrections are made.

I honestly think that the questionnairre could be a lot more detailed. It should include a lot more questions, including feedback about the effectiveness of GSCASH.
