One of U employee will participate as a clinical study subject.  He will receive a gift card as subject payment. The gift card is less than $600.   Should I fill out a payroll form and report to payroll service since he is a U of M employee?

I would not notify Payroll as I can not locate a direction in policy to do so.  Per the policy on Paying Human Subjects (including employees), notification to Disbursements is required if payment or value of payment is $600 or more but doesn't indicate any special circumstances for employees as subjects.
