Summer Camp Paperwork DUE

Please make sure to submit all required summer paperwork no later than 06/27. If we do not have prior to camp, you will not be allowed to attend,    I will be at tonight's meeting to collect documents.

Remember- parts A, B and C of the medical forms and also a copy of your medical insurance cards.

Please let me know if you have any questions. As a reminder, adults attending will also need these forms complete. If you had a physical  just prior to winter camp, your medical paperwork may still be valid but I will need to confirm and still need the copies of the documents in hand.
