"Never ending" poll

Hi all

I have a frustrating problem! I want to use / create an app / website / poll similar to something you would see in an airport or Home Bargains! I have attached a photo to try and explain what I am on about!

I want to have an iPad in front of the young children in my school... then I simply want two choices in front of them (Sandwich or Hot dinner with pictures!) 

I want them to be able to touch it, the software register the vote then return back to the original screen... 

I have tried this in a range of different websites, but without success yet! 

ANY help would be very welcomed.

Could you not add an image to each of the choices in a Google Form. They could select their name from a drop-down question. Make both questions compulsory.
