Updated msh4 reader

Hi all

I have updated the .msh reader to handle version 4. The outputs using
MESH_POST (or MESH_FLUX) are still in version 2.2 though.

Please always update wasora before updating fino and/or milonga.
Remember you can watch the repositories to get email notifications of
the commits. You need to create an account in bitbucket and the choose
"watch the repository" from the top-right menu.

Those who have forks, please merge the new changes as some changes in
the ABI has changed a little bit.
