Hello everyone! I am attaching a copy of the BSA Activity Medical Form. Anyone attending this weekend's camping trip needs to fill out a form for your scout(s). They will need to be in your possession while at the campgrounds in the event that there is a medical emergency. Although each parent is responsible to stay with their scout at all times, in the event that a scout is separated from their parent and they get sick or injured, the completed medical form will need to be readily available.
