Summer camp Pack list/misc. documents

Attached are the optional documents that are due July 1st.  I will be at the church from 6pm to 7pm on July 1st.  If we do not have these by July 1st there is no guarantee that they can be accommodated by the camp.  I have also attached a packing list for anyone that would like to use as a guide.
