User-defined Fields and Notes Field Changes

Hello Everyone,

I've made many changes to the HamLog.it website this past week.  The end result is much faster load time when there are several devices tied to the account and user-defined fields.

In Prefs there are 4 new input fields to put the ADIF key of the field you'd like to add.  There are also checkboxes to show the data saved with a contact in the main contacts table.  The ADIF key is also saved with a contact to account for future changes to what you want to be in that field.  For example,  today you want User Field #1 to be a SOTA summit and then create some contacts with that data.  Tomorrow you want to rename that field to an IOTA designator and add some more contacts.  When you export ADIF the SOTA and IOTA keys will be present with their respective contacts.

I've also increased the max length of the notes field to 100 characters.
