Does anyone have a form to fill out that is a one time thing for the whole school year?

On AET, under a student's profile tab, if they click "Profile, Classes, Contacts & Approvals" and then "Student Agreement / Parent Approval & Emergency Info" there is a generic one that students can send to parents, and then you have access to it in AET.
