Capturing and viewing vaccinations

How are other projects capturing vaccinations?

At PIH Rwanda (IMB), we are using an htmlform flowsheet to edit/view vaccinations in a grid -- showing what vaccinations have been completed with date, and what is still needed.  I recall that someone worked on a vaccination module, and there was a mobile application (ODK?).  I am pondering a quick implement using an htmlform along with the lookup velocity function to show past vaccinations.   This would be used on computers in a hospital environment, so mobile devices aren't a good fit.

It was MVP. We have the EzVac module which works with the CIEL dictionary, captures data using an xform, and provides age/gender specific recommendations based on a schedule which can be configured.

It would be helpful to see a demo, documentation, screenshots, and/or a downloadable module.  Is this currently in use?  What version of OpenMRS is required?
