At Partners In Health (PIH), we have been managing concepts and metadata for multiple implementations (Haiti, Rwanda, Lesotho, Peru etc.)  This is an alternative to using the entire CIEL concept dictionary.  We do this with one "golden" metadata server which is used with the metadata module to manage concepts and other metadata for the implementations.  One of our latest projects is with the Rwanda Ministry of Health, so that we can share forms and metadata between the Ministry implementations and those in 3 health districts where PIH helps with healthcare strengthening.  This is ongoing work, but we've created a wiki page with general guidelines and tips for managing an OpenMRS metadata server.  It provides suggestions for concepts and htmlforms.

We have also written many mysql and groovy scripts for performing each of concept cleanup tasks along with adding reference terms.  These will be documented in a public forum.  Some of the groovy scripts have already been added to the OpenMRS wiki page with the rest of the groovy scripts (https://wiki.openmrs.org/display/docs/Groovy+Module).

I'd love to hear feedback from others about how they manage their metadata and if they do this for multiple implementations.  Please feel free to add your own guidelines to the wiki document.
