Not sure if community members are familiar with the OpenMRS Form Bank.  The idea is that anyone can contribute, borrow, or learn from existing forms.  All the forms are currently htmlforms, but all forms are welcome.  Packages are built with metadata sharing, so they include all the underlying concepts and metadata.  We recently regenerated some of PIH's htmlforms using OpenMRS 1.9 and the latest metadata modules.  Forms created using OpenMRS 1.6 continue to be available.  In addition to the electronic form, copies of the paper forms are also available on the page.

Hi Ellen thanks for a reminder about this great resource. I'll post some of our primary care forms soon.

Can you think of an easy way to share the HTML from the form in addition to a full metadata package? I sometimes find importing metadata package to be problematic but having a quick way to glance at the HTML would be really helpful. 

I really like having the paper forms but it would be interesting to see how some of them rendered, especially the ones that contain a flowsheet. Would it possible to post screenshots or pdf prints?
