I have some Infopath forms for HIV if you would like them, but agree with Alex that Infopath has mostly been abandoned.  In addition to HTML Forms, there are XForms (which are particularly good if you want to support mobile apps) and HTML5 running against the REST API (Thoughtworks/JSS).
