Building a custom tab for observations

Hello,

I want to add a custom tab in the patient dashboard in order to be able to save observations related to specific issues e.g. gynaecological issues etc.
Is there an easier way to save/view/edit observations in a custom tab, than writing custom jsp code using forms and then validate and save the observations 
using openmrs' API in my PortletController ?
Basically what I am wondering is can I use an html form instead of the solution described above, as this would save me lots of work ?
There is just a broken link on this subject on the wiki page (How to Embed HTML Form (generated by HTML Form Entry module) in a custom module)

I'd appreciate any input as I am kind of stuck at the moment

Charis,

I haven't been working within the OpenMRS 1.x UI in quite some time, so I don't remember exactly how to go about this.

Adding the OpenMRS help desk--is there anyway we can determine why this link no longer works?  I believe that it did work at one point... the link can be found here:
