updated waiver form for USA Cycling

Hi everyone.  I just got notified that the USA Cycling waiver has changed from the previous editions.  It has already been updated on BikeReg before the season started; so nothing to worry about there:  But for those of you that do paper waivers for your day of entries, I've included the new version with additional space to capture addresses, e-mail, phone numbers, and other important information.
