Updated Mass Revocation wiki page

All,

I have updated the Mass Revocation Events (MRE) wiki page with a new section that outlines expected plan contents, and I also revised the plan template by relabeling section 4 to "Classification" and renumbering "Definition and Declaration of an MRE" to section 4.1 and adding a section 4.2 "Customer Contact Information" and section 4.3 "Identification of Manual and Automated Processes".
