CCADB: Added Instructions for New User and Password Reset

This is just FYI that I updated the "For CAs" tab of www.ccadb.org to add New User & Password Reset instructions. I also did some minor cleanup in Getting Started.

As always, I appreciate constructive feedback and questions to help improve the CCADB and corresponding instructions.
