Reminder: Complete the new Miami County Continuum of Care membership form

If you have not already done so, please complete the new Miami County Continuum of Care membership form (https://forms.gle/akF97Xyk6RTF9nN7A) to update your contact information. If you know someone who is not currently receiving Collaborators emails or attending the Continuum meetings who would like to, please share the link or QR code with them. I will plan to send a contact list out to all members at the beginning of January. I hope the list will help to give everyone a direct contact at each agency for communication needs outside of our regularly scheduled meetings.
