Dear PCHS Parents/Guardians,

If you have a Junior or Senior attending prom, or a Freshman or Sophomore who is going with a Junior or Senior, please read below!

Please complete this  Allergy Form if your student has any food allergies and is attending prom. We will provide a list to the event center so they can make sure to accommodate. 

Because students often go out to dinner before prom, please note we are serving appetizers only that include: Salad bar, fruit, chicken strips, parmesan fries, breadsticks, mozzarella sticks, and desserts that include cupcakes, cakepops, brownies, cookies, and ice cream floats.
