The Gemma Project is excited to announce that we are currently hiring for a Continuing Care Coordinator – Bilingual English/Spanish to join our team in Contra Costa County.

The Gemma Project is a female-led, trauma- and gender-responsive nonprofit that provides reentry programming for incarcerated and formerly incarcerated women. Our work begins inside local jails and continues in the community through intensive care coordination, system navigation, and reentry support.

The Continuing Care Coordinator will play a vital role in bridging support for women transitioning from custody to community. This position works closely with participants to co-create Wellness & Reentry Plans prior to release, provide system navigation and ongoing reentry support, facilitate groups, and collaborate with community and county partners to ensure a safe, structured, and compassionate reentry process.

Details:
  • Full-time position with health, dental, vision, retirement, and PTO benefits.
  • Approximately 30% of time will be spent working inside jail facilities.
  • Individuals with lived experience of justice system involvement are strongly encouraged to apply.
