Updates to Public View; Pellucid Friendly Names

Pellucid Public Hospitals View (public) has been updated with
additional measures and additional reporting entities. All measures
can now be browsed on the wiki at
http://wiki.ipro.org/index.php/Public_view

Some highlights:
- addition of county and state to public.hospital_values - now you
can select values directly out of public by state or county.
- addition of aggregate entities (ZIP and county) to public.entities
- addition of US census data

Pellucid 2.0 Changes

pellucid2.measures has been modified for a handful of measures - AHRQ
PSIs now contain friendlier name_friendly entries
