Didn't perform a detailed review but a couple of things apparent on a cursory read:
- be clear that "Pathology" (as in anatomical histopathology or clinical laboratory pathology) is not in scope at all, because there has been no participation by PaLM (who have been made aware of your indiscretion), and pathology reporting is a specialized field (with specialized vendors and practice standards) about which IHE RadTech participants probably know little if anything (two adjacent paragraphs in the introduction contradict each other in this respect)
- use of the term "pathology" to mean "condition" or "disease" should probably be changed throughout (e.g., though "no pathology present" may be a colloquial US radiologist's expression, in the absence of a specimen under a microscope it should probably be "no disease present" or "no condition present" - there are multiple occurrences of this sort of thing); when you really mean "anatomical histopathology" or "clinical laboratory" result (as prior information or confirmation information), if ever, say so by describing it fully
- don't use SNOMED CT post-coordinated expressions, since in the absence of constraints they undermine interoperability by requiring additional parsing and an ontology to understand as opposed to just recognize; pre-coordinated codes are easy to come by for any typical use case
- some of your hyperlinked references are not clean but have been indirected through a urldefense link, which seems inappropriate
- the absence of examples in the PC draft makes it difficult to evaluate the utility or technical correctness of the supplement, since the exercise of making examples is a good testing process
