Request for Certificate of Acceptance of Data Use Terms

Dear support team,

According to the HCP-Aging agreement, a certificate evidencing acceptance of the Data Use Terms can be obtained upon request. I am unsure of the proper process for requesting this certificate, or whether it should be done through the BALSA website.

Could you please provide me with the certificate, as I need it for submission to my institutional review board?
