Great 😊 We’d like to review our Disability Verification Form (original form) with you. It hasn’t been updated since the start of CES.
We have recently had an issue of evaluators/diagnosticians signing the form but indicating they have not personally evaluated or treated the patient. So we added highlighted text to try to address that (see attached).
We wanted to confirm disease of acquired immunodeficiency syndrome was still a HUD disabling condition.
Happy to check in and discuss if that works best.

Thank you! I thought immunodeficiency syndrome (AIDS) was – but saw something in an online homeless group that contradicted that. After that the main thing is your feedback on the highlighted additions we added to the form that we believe address the issue that some evaluators/diagnosticians were signing the form but indicating they have not personally evaluated or treated the patient. Timeline is hard -sooner is always better 😉 But ideally by Dec 15 so we can add to trainings for the new year. All the best, Emily
