Hi Meadow,

Moving you to bcc to spare your inbox.
Hi Emily,

It has been a little while since we last connected.  I hope you are doing well.  Yes, our contract with you includes TA so feel free to respond with your inquiry.  I have cc'd the Santa Barbara HB team.  We might not be able to get back to you until after Thanksgiving but let us know how we can help.

Best,
Kelly

Hi Meadow,

Hope you’re well! We had a question about our disability verification form. Jett thought we still had some YAB TA available. If so we’d like to run it by you. Let me know – thank you 😊

All the best,

Hi all,

Great 😊 We’d like to review our Disability Verification Form (original form) with you. It hasn’t been updated since the start of CES.

We have recently had an issue of evaluators/diagnosticians signing the form but indicating they have not personally evaluated or treated the patient. So we added highlighted text to try to address that (see attached).

We wanted to confirm disease of acquired immunodeficiency syndrome was still a HUD disabling condition.

Happy to check in and discuss if that works best.

All the best,

Emily Allen

Hello Emily -

I can review your Disability Verification Form and get back to you with any suggestions we may have for updating it.

In the meantime, I can confirm that acquired immunodeficiency syndrome (AIDS) or "any condition arising from the etiologic agency for acquired immunodeficiency syndrome" (HIV) are still considered a HUD disabling condition (per the HEARTH Act). Here is a helpful resource from HUD regarding the definition of disability: https://files.hudexchange.info/resources/documents/Virtual-Binders-At-A-Glance-Disability-Definition.pdf

Please let us know if you have any other specific questions. It would also be helpful to know if you have a timeline for when you need any suggested updates for the Disability Verification Form.

Take care,

Devra

Thank you! I thought immunodeficiency syndrome (AIDS) was – but saw something in an online homeless group that contradicted that. After that the main thing is your feedback on the highlighted additions we added to the form that we believe address the issue that some evaluators/diagnosticians were  signing the form but indicating they have not personally evaluated or treated the patient. Timeline is hard -sooner is always better 😉 But ideally by Dec 15 so we can add to trainings for the new year. All the best, Emily

Thanks Emily -

We should definitely be able to get you a more in-depth review and feedback on the form by December 15th, and sooner if possible.

Take care,

Devra
